Scheduling
At Arrow Endodontics, we see patients Monday thru Thursday 9am-6pm and Fridays 7am-3pm. We understand that you would like to have your work done as soon as possible and we will schedule your appointment as promptly as we can. If you have pain or an emergency situation, every attempt will be made to see you on the same day.
At Arrow Endodontics, we try our best to stay on schedule to minimize your waiting. However, due to the fact that we take the time to provide each patient with the best endodontic service, we find that various circumstances may lengthen the time allocated for a procedure. We also find that emergency cases can also arise and cause delays. We appreciate your understanding and patience.
Please feel free to call 909-394-5500 with any questions or to schedule an appointment.
First Visit
Your initial appointment with Dr. Patel will consist of an evaluation explaining your diagnosis and treatment options. During this visit, we will review and discuss your chief complaint, detailed tooth pain history, medical and past dental history as well as any dental phobia (if any). In addition, a radiographic exam and pulp vitality tests will be performed to analyze the pulpal and periradicular status of the tooth in question. We may require different angled x-rays to obtain all the necessary details. Hence, even though you may bring your own set of x-rays, it is necessary to take additional 2 dimensional images in our office to obtain all the necessary information to help us achieve the right diagnosis. Sometimes a 3 dimensional Cone Beam scan is recommended as well.
Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please bring to your appointment:
• Referral form from your dentist
• Dental insurance information
• Accurate list of current medications
Please inform our office prior to your appointment if you have a medical condition that may be of concern prior to treatment.
Financial Policy
We would like to provide you with information about our office financial policy before your visit. Unlike your general dentist, as a specialty practice, we may only see you as our patient for one treatment visit. Therefore, payment is due at the time services are rendered.
Our office welcomes many types of dental insurance, and our front office staff will be happy to assist you in determining what type of benefits are available to you through your insurance plan. As a courtesy, if you have dental insurance we will bill your carrier, provide documentation, claim forms, radiographs, and treatment narratives. We will request that you pay your estimated co-payment at the time of service.
We accept cash, checks, Visa, MasterCard, Discover and American Express. We also offer interest free financing and payment plans through CareCredit.
New Patient Forms
Our office is a paperless office so all the forms are completed on the computer. Our staff will personally assist you in filling all the forms during your first visit.
